茲應票據持有人×××的要求（票據原件附后），本公證員于××××年×月×日向票據付款人×××提示原票據，并要求其付款，遭其拒絕。
據此，本公證員特立此證書證實該票據項下付款確實遭到匯票付款人拒絕。
